All entering, new students are required to provide proof of immunization to the Student Health and Wellness Center (SHWC). If you have not completed this form, it is important for you to attend to it as soon as possible, so you can avoid a hold on your registration. If the form was completed improperly or if your immunizations are out of date, you will not be permitted to move into the Residence Hall (if applicable) or register for classes. We encourage you to make and keep a copy of this form for your records.

There is a three-step process to complete the verfication of immunization on the health portal. For health forms and instructions, go to https://studentaffairs.jhu.edu/student-health/incoming-students/. YOUR HEALTH FORM MUST BE REVIEWED AND CLEARED BY THE STUDENT HEALTH CENTER NO LATER THAN JULY 1st TO AVOID ANY DELAYS IN REGISTRATION OR MOVE-IN.

Health Insurance Coverage

The Johns Hopkins University requires all full-time students to carry comprehensive health insurance and offers a student health insurance plan administered by Consolidated Health Plans as the standard for adequate health insurance coverage. For details, go to your SIS account. Consideration of the university’s health plan requires your careful attention, so we will be sending you more detailed information about it via email. Note that charges for the school’s insurance will appear on your bill. However, you may waive the school’s program upon proof of comparable, alternative insurance no later than September 15, 2017 via the your SIS account. Waivers must be completed every year. Once we have been notified that you have adequate health insurance coverage, the charges will be removed or refunded.
